Common approaches for small employers
Traditional group coverage
The employer selects group plans, defines eligibility, and generally contributes toward employee premiums. Participation, contribution, enrollment, carrier, and state rules affect availability.
Individual coverage reimbursement arrangements
An ICHRA may reimburse eligible employees for qualifying individual coverage and expenses under defined classes and rules. A QSEHRA may be available to certain eligible small employers that do not offer a group health plan. These arrangements require formal documents, notices, substantiation, and coordinated tax treatment.
Voluntary and supplemental benefits
Group life, disability, dental, vision, accident, critical illness, and hospital indemnity benefits may broaden a package. Employers should distinguish comprehensive medical coverage from limited products.
Questions to answer before requesting proposals
- Which employees are eligible, and when does coverage begin?
- How much can the business contribute consistently?
- Where do employees live and receive care?
- Are specific doctors, hospitals, or prescriptions important?
- Who handles enrollment, payroll changes, notices, and employee questions?
- What happens after a hire, departure, hour reduction, or Medicare eligibility?
Compare more than rates
Review networks, prescriptions, deductibles, out-of-pocket limits, contributions, employee cost, participation, renewal history, service, and administrative workload.
Benefits questions
Does a small business have to offer health insurance?
Requirements depend on employer size and circumstances. Obtain current legal and tax guidance.
Can different employee groups receive different benefits?
Some arrangements permit defined classes, but nondiscrimination, affordability, notice, and other rules may apply.
Can employees keep individual plans?
That depends on the approach. An ICHRA or QSEHRA may coordinate with qualifying individual coverage under specific rules.
When should benefits be reviewed?
Start before the desired effective or renewal date to allow time for census review, proposals, notices, and enrollment.
